504 meetings will be held either in person or through phone conference.  Annual reviews will be held on an as needed basis.  Three-year Re-evaluations will be held based on the student’s qualification date.  When the 504 meeting is scheduled, you will be notified as to whether it will be conducted in person or by phone conference.

If you believe your student might qualify for 504 services, you should contact the School Counselor.  The School Counselor will then discuss your concerns and the 504 process.
