Health Forms

These Medical Care Plans are needed each school year.

  • Asthma Care Plan
  • Allergy Emergency Action Plan  – Note to users of Assistive Technology – only some fields are fillable by the parent.  Form should be printed and brought to your healthcare provider. 
  • Seizure Action Plan (SAP) English and Spanish  (External Link to Epilepsy Foundation.)
  • Conscience Objection Online form
  • Request for medication to be administered in school – Printable form to bring to your healthcare provider. Note for users of Assistive Technolgy: this is not a parent-fillable form.  It should be printed as-is and brought to your child’s healthcare provider.  (Document can still be fully read with a screen reader.) 
  • Migraine Action Plan – Note to users of Assistive Technology – only some fields are fillable by the parent.  Form should be printed and brought to your healthcare provider. 

Just a reminder that your child's physician needs to fill out and sign the care plan.  Forms are available in the Nurse’s Office. If there are any changes to your child’s medication during the year, please let me know so that I can update your child’s records. Any changes will require a new order from your child’s physician and a new Medication Permission slip. It is the responsibility of the parent/legal guardian to provide all prescribed medications, supplies and equipment to the school. If an emergency arises before receiving the necessary items, the nurse will follow first aid procedures and 911 will be called.
